Medicare

How to get a Dietitian referral (Medicare rebate)

To see a Dietitian with a Medicare rebate, you need a care plan from your GP.

Steps:

  1. Book a longer appointment with your GP (for a Care Plan). 
  2. Your GP will assess your health and create a Chronic Disease Management (CDM) Plan. 
  3. As part of this plan, your GP can refer you to a Dietitian. 
  4. You can receive up to 5 allied health visits per year (shared between services). 
  5. Bring your referral to your Dietitian appointment to claim the Medicare rebate. 

Good to know:

  • There may be a gap fee depending on the clinic. 
  • Your GP manages and reviews your care plan. 

DVA

How to get a Dietitian referral (DVA clients)

If you are a Department of Veterans’ Affairs (DVA) card holder, you may be eligible for Dietitian services.Steps:

  1. Visit your GP or specialist. 
  2. Ask them to complete a DVA referral form (D904) for Dietitian services. 
  3. Once the form is completed, you can book an appointment with a Dietitian. 
  4. Provide your DVA card and referral at your appointment. 

Good to know:

  • Most Dietitian services are fully covered for eligible DVA clients. 
  • The number of visits depends on your clinical needs and approval.
